Transplant Coordinator Update
September 2008
We recognize that our coordinator staff has undergone significant turnover during the last 18 months and we appreciate all of your patience during this time of transition. We are undertaking a series of changes in order to create a system that better meets the needs of our patients. The transplant nurse coordinators have recently reorganized their care model to focus upon your special needs before and after transplant. This is a model that is successful at many transplant centers across the country. We have 2 coordinator positions available at this time. We will announce our new coordinators as they finish orientation and begin to independently care for patients. The current coordinator team is listed below.

Collaborating with Your Transplant Coordinator
The nurse coordinators provide care in the clinic, in the hospital and through telephone conversations with you. In order to continue to provide efficient and effective care to our patients, we would like you to consider the following when contacting the nurse coordinators:
1. Refill requests are most accurate when requested by your pharmacy. Please call your pharmacy to request refills; your pharmacy will send us a request. You may also leave a message on the Prescription Refill Line. Refills should not be called to the on-call coordinator.
2. Please remember to keep 7 days worth of medication on hand in case there is a delay in filling your prescription. Many medicines require prior authorization by your insurer and this process may take 3-5 days.
3. If you have not heard from your coordinator regarding your lab results within 1 week we probably have not received the results. Please call the lab and ask for them to be resent and call the coordinator to ask them to look for the labs.
4. General questions including those related to activity, new medications, physical limitations and laboratory results are messages that should be left for the coordinators on voice mail. Voice mail is checked frequently during the day. Our goal is to return calls within 1 business day. Please leave your name and phone number each time you call.
5. Routine healthcare maintenance needs should be directed to your local provider. This includes diabetes care, mammograms, pap smears, and prostate exams. If you do not have a primary doctor, we would like you to establish care with one as soon as possible.
Paging the Transplant Coordinator
Paging the transplant coordinator should be reserved for urgent issues only. If you must, to contact the Nurse Coordinator during the business day for an urgent issue that cannot be left on the office voice mail, you should request to speak to your coordinator when you speak with the paging operator. Your coordinator will answer your call when he or she is available at work. After business hours and on the weekends, your call will be forwarded to the Lung Transplant Coordinator on-call who will assist you.
Clinic Redesigned for Service Improvement
Beginning October 1, 2008, the Lung Transplant Clinic patients will be give a specific time to attend clinic. The times are 1:00 p.m., 2:20 p.m. And 4:00 p.m. Your time will be provided to you in your clinic confirmation letter. Please plan your clinic visit so you arrive in the waiting area 15 minutes before your physician appointment time. Patients who must arrive for morning lab work and studies, will be able to plan their return to clinic to prevent prolonged waiting periods. This is a cooperative effort between the clinic and the transplant program. We hope you will find it helpful as well.
Please disregard telephone and email reminders that are provided automatically. The automated systems do not consider planned studies and blood work. Please refer to the times in the confirmation letter provided by the Lung Transplant Office. You may also call our office and speak to one of our medical secretaries for information about your appointment.
